	I have one of the following certificates in hairdressing: NVQ or SVQ Level 2; City and Guilds; SCOTVEC; Incorporated Guild of Hairdressers. Apprenticeship papers are also acceptable.
	I have one of the following certificates in hairdressing: NVQ or SVQ Level 3; City and Guilds or SCOTVEC; Advance Hairdressing Certificate - copy enclosed.
	I have at least six years experience as a practising hairdresser which may include period(s) under hairdressing training. This minimum of six years experience MUST be certified by someone in an official position on their headed notepaper.
	I have at least nine years experience as a practising hairdresser which may include period(s) under training. This minimum of nine years experience MUST be certified by someone in an official position on their headed notepaper.

Declaration

I hereby apply for registration and enclose the appropriate fee and supporting paperwork in accordance with the Hairdressers (Registration) Act. Should this application be unsuccessful, I understand the fee will be returned. Should my application be successful, I understand my name will be added to the official register of hairdressers for a period of twelve months.

I also understand a fee will apply for each year, or part year, my name is retained on the register. If, at any future time, I wish my name to be removed from the Register, I undertake to advise the Hairdressing Council in writing not less than two months prior to the date of expiry of the certificate of registration held by me at the time.

Registration Fee

Fees may be paid by one of the following four methods;

	1. £37 - Cheque or credit card (includes SRH Certificate)
	2. £45 - Cheque or credit card (as 1 above plus frame)
	3. £34 - Direct Debit (includes SRH Certificate)*
	4. £42 - Direct Debit (as 3 above plus frame)*

* Include discounts for use of Direct Debit scheme; subsequent fees paid by Direct Debit will remain discounted. If you want to pay by Direct Debit please complete and print off the the Direct Debit Mandate.
